Liver transplantation surgery is the process of replacing a healthy liver in the place of a damaged liver/failed liver with a healthy liver/portion of a liver from a donor. Liver transplant surgery is done on patients in end-stage chronic liver failure, acute liver failure and liver cancer with no response to other treatments.

With advanced and state-of-the-art technology, most patients lead a normal life within 3 to 4 months of having the surgery. After surgery, routine health check-ups, proper medication and a healthy lifestyle can aid the patient to increase their lifespan. However, there are slight possibilities of infections, bleeding and minor surgeries after the surgery.

The transplantation procedure takes place when a patient in end-stage liver failure is listed on the organ donation waiting list and they find their donor. It could either be a living donor or deceased donor. In a living donor transplant, the donor donates 20 to 30% of their liver to the recipient. In a deceased donor transplant, patients are offered organs based on the state-wise organ donation list.

At MGM Healthcare, the decision to perform a liver transplantation surgery is taken only if the process enhances their chances of survival. Mandatory clinical tests, right from blood compatibility to clinical assessment from the concerned consultants, all these contribute to the eligibility criteria for liver transplant used to determine if a person in end-stage liver failure is fit for surgery. Evaluation of the heart, lungs and kidneys is also necessary for liver transplantation surgery. The patients must pass all the eligibility criteria for liver transplantation.

Post Operative Care
Post-surgery care plays a significant role in treating patients. The patient is usually taken care of in the liver transplant ICU for 4 to 5 days. They will be under 24X7 observation and the necessary clinical tests, like blood tests and ultrasound tests, ensure their overall wellness. Once the patient shows affirmative signs of organ acceptance, the patient is shifted to the general ward. Patients typically stay in the hospital for about 3-4 weeks after a transplant. Counselling on lifestyle changes and medication will be provided to the patient during their hospital stay.

Success Rate
India has the highest success rate for liver transplants, where the country is one of the best places for liver transplantation. The liver transplantation success rate is estimated to be 95%, depending on their post-surgical recovery. A significant increase in the life span of the patient happens after the surgery. MGM Healthcare boasts an impressively high success rate for liver transplants in Chennai and India. It is one of the best liver transplant hospitals in India, equipped with the best liver transplant surgeons.

The liver is a vital organ in the body and the largest internal organ that performs many known and unknown essential functions of the body. It is the only organ in our body that has regenerative capabilities. Measuring an average of 5.5 inches, the liver is reddish-brown as it is saturated in blood. It is situated above and to the left of the stomach and below the lungs. The liver consists of two lobes, a larger right lobe and a smaller left lobe.

The liver is responsible for

  • Processing nutrients, medications and hormones
  • Producing bile, which absorb fats, cholesterol and vitamins
  • Producing proteins that help the blood clot
  • Removing bacteria and toxins from the blood
  • Preventing infection and regulating the body’s immune responses

The liver is attached to the gallbladder. It secretes bile into the hepatic ducts. The pancreas also secretes digestive juices. The liver, gallbladder, hepatic ducts and pancreas together form the hepato-pancreato-biliary system.

Acute liver failure is a rapid loss of liver function where the organ loses its ability to function over a short period of time. Liver failure majorly occurs in a healthy person who has no pre-existing liver illness. Hepatitis virus or overdose of drugs like acetaminophen are the prominent cause of liver failure. This condition needs immediate treatment as it results in profuse bleeding and liver damage, thereby increasing pressure in the brain. Acute liver failure treatment in India is extensively carried out with high success at MGM Healthcare.

Deceased Donor Liver Transplantation or Cadaver Donor Liver Transplantation is a surgery where a portion of the liver is removed from a deceased person and transplanted into the recipient. Deceased donor liver transplantation is performed on a patient who is in the end-stage of liver failure.

Living donor liver transplantation is a surgery where a portion of the liver is removed from a living person and transplanted to the recipient. Living donor liver transplant surgery is performed on a patient who is in the end-stage liver failure. The donors are usually first-line blood relatives, their spouse or friends who are a match. They willingly donate a portion of their liver to the patient.

Not every patient with a liver condition requires a liver transplant. Many respond well to other forms of treatment like medication. In some cases, treating the overlying cause of the liver failure can restore the functioning of the liver.

Patients who are diagnosed with the following may require a liver transplant:

  • irreversible liver failure
  • those with liver cancer untreatable by other modalities
  • those with metabolic liver diseases
  • acute poisoning that has resulted in permanent damage

Once a patient is declared to be in end-stage liver failure, the doctor needs to assess if the patient fits the eligibility criteria for a liver transplant. While each transplant centre has its own criteria, they are all by and large the same across centres in different countries. Age is not a contraindication.

The following must be determined during the assessment:

  • Is the patient healthy or fit enough to have the surgery and tolerate the lifelong post-transplant medication?
  • Do they have any medical conditions, like extensive cancer or uncontrolled infection, that would interfere with the success of the transplant?
  • Are they willing to follow the post-transplant medication and lifestyle modifications that will be necessary to ensure the continued success of the transplant?

The evaluation may include nutrition counselling, psychological evaluation and addiction counselling (if the liver damage is caused by excessive and sustained  consumption of alcohol and drugs). Once the evaluation is complete and the patient is found eligible, they are placed on the transplant list.
